The article presents studies aimed at transforming invariant means of multi-level optimization modeling in the management of a geographically distributed organizational system with personalized subjects in relation to the distribution of resources for drug provision and medical care for patients suffering from rare diseases. It is shown that the management of the resource distribution process requires synchronization within a single route for the allocation and use of funds from the federal and regional budgets on the basis of the existing regulatory framework. In order to move to the transformation of invariant optimization models, the features of functioning and management in the personalized organizational system of pharmaceutical and medical care for patients suffering from rare diseases are preliminarily substantiated. The main features are: a multi-level nature of the distribution of a centralized resource; availability of information resources in the form of registers, registers, database of applications; monitoring of performance indicators and monthly reports of the regions; automation of the integration model of management using digital information resources and GIS-oriented technologies. Taking into account the features of the organizational system under study, changes were made in the formation of a problem-oriented process of multi-level optimization modeling: in the description of sets that determine the inter-level interaction of management bodies and patients; in the form of additions and changes to the formalized description of optimization tasks and procedures for making management decisions; with the unification of an integration model for managing the distribution of a centralized resource of administrative decisions and means of their intelligent support based on an optimization approach.
